- Certificate of concerned F.P. shop to the effect to the effect that entry of deletion is made in Card Register of FPS with the reason of deletion.
- Death Certificate in case of deletion of the name from the card.

Issue of the Duplicate Ration Card-
Eligibility–
Who have Lost the Ration Card can apply for the duplicate Ration Card.
Procedure & Documents-
- Submit the Application form and required documents as listed below to the authorized person/ office.
- Certificate of the House Number from concerned Panchayat.
- Affidavit from Executive Magistrate on the case of loss of original Ration Card.
- Certificate from concerned F.P. shop to the effect that the card stood registered with F.P. Shop with the entry of Ration Card.
- Two passport size photos of the head of the family.
- Applicable Fee- Rs. 10/-
- Expected Time Period- 7 days
Change of the Address/ Change of the FPS :
Eligibility– In case residence area has changed
Time Period– 7 Days
